The treatment of fractures and nonunions with dynamic axial fixator DAF

Abstract

In this study, the results of 41 humerus, tibia, femur and radius fractures and delayed uninns and nonunions diagnosed in patients of Istanbul Hospital and treated with the dynamic axial fixator (DAF) were reviewed. There were 28 male and 11 female patients with the average age of 31 years. Fractures were closed in ten of the cases; of 12 cases, 8 patients had hypertrophic and 4 athrophic nonunions. Patients were surveyed for 25 months in avarage (18-32 months). Operation time was assessed to be betwwen 25-45 minutes. In nonunions the date of experienced trauma was 8 to 48 months. AIl of the cases were operated under general anesthesia with scopy control, on a fracture table. The axial fixator was dynamised for an averge of 5 to 7 weeks. In 20 of the 194 pins we used, pin-tract infection was encountered which was Iater treated. For all patients, the overall results were good. it can be concluded that DAF is a safe, easy, useful and effective method in managing both fresh and old fractures which are either open or closed fractures.

Özet

SSK İstanbul Hastanesi 1. Ortopedi ve Travmatoloji kliniği nde Eylül 1990-Kasım 1991 tarihleri arasında, 39 hastanın 41 kırık ve nonunionlan dinamik aksiyel fiksatör (DAF) ile tedavi edildi. Hastaların 28 i erkek, 11 i kadın olup, ortalama yaş 31 (5-60 yıl) yıldır. Ortalama izleme süresi 25 (18-32 ay) aydır. Olgulardan 10 u kapalı kırık, 19 u açık kırık olup, nonunionlann 8 i hipertrofik, 4 ü atrofik tiptir. Ortalama 5-7 haftada dinamizasyona başlanarak tüm olgularda kaynama sağlanmıştır.8ir femur kırığında refraktür ve kullandığımız 194 çivinin de 20 tanesinde tedavi ile iyileştirebildiğimiz çivi yolu infeksiyonu ile karşılaşılmıştır. DAF ile açık kırık, çok parçalı kapalı kırıklar ve nonunionlar başarılıl şekilde tedavi edilebilirler.
