The role of posterior fusion and instrumentation after anterior radical debridement and fusion in the surgical treatment of Pott’s disease

Abstract

Introduction: Long periods of immobilization and progressive kyphosis and graft fai/ure are the major post-op problems encountered af ter anterior radical surgical treatment for tuberculosis of the spine. Posterior fusion and instrumentation can be the effective solution for these problems. Effectiveness of posterior fusion and instrumentation is investigated in this study based on cases with anterior procedure only and wiıh combined procedures. Material and Method: 127 cases of tuberculosis of the spine were surgically treated between 1987 and 1995. They all had eiıher one or more of conditions like spinal cord compression and neumlogic deficit, vertebral body collapse and kyphosis or wide paravertebral abscess unresponsive to medical treatment. Of these 57 had only the anterior radical procedure between the years of 1987 and 1993. 70 cases had posterior instrumentation and fusion following the anterior procedure between the years of 1991 and 1995 . In about 213(81 patients) autogenous iliac strut graft and 113 (40 patients) autogenous fibular strut graft (cases with more than two level involvement) were used along with rib-grafts af ter anterior debridement. 30 patients had MRf of the whole spinal column and 10% had multi/evel lesions which were not seen at plain X-rays and caused neurologic deficit. Results: 21 of 57 patients who had anterior procedure only demonstrated a postop increase of kyphosis more than 10°. fncreased kyphosis was due to graft slippage in 3, resorption in 2 and subsidence in 16 patients. No such increase or graft failure was noted in cases of combined procedure and posterior instrumentation. The difference in terms of kyphosis was found to be statistical/y significant (p=0.047). 14 of 127 patients were either paraplegic or paraparesic at presentation and neurologic recovery started in one to six weeks af ter surgery. Conclusion: Anterior radical debridernent and strut-graft is the golden standard in the surgical treatment of spinal tuberculosis but it should always be accompanied by posterior instrumentation and fusion to shorten immobilizaıion period and hospital stay, obtain good lind lasting correction of kyphosis and prevent jurther col/apse and graft failure. 

Özet

Giriş: Omurga tüberkülozunda anterior radikal cerrahi tedavi sonrasında uzun süreli immobilizasyon, greft yetersizliği ve ilerleyici kifoz karşılaşılan en büyük problemlerdir. Çalışmamızda, bu problemliler açısın-dan posterior füzyon ve enstrümantasyonun etkinliği araştırılmıştır.\nHastalar ve yöntem: 1987 ve 1995 yılları arasında omurga tüberkülozu sebebiyle, 127 olguya cerrahi te-davi uygulanmıştır. Tüm olgularda medulla spinalis basısı ve nörolojik defisit, omurgada çökme ve kifoz veya ilaç tedavisine yetersiz yanıt gibi durumların biri veya fazlası bulunmaktaydı. Olguların 57 sine 1987-1993 yılları arasında sadece anterior girişim, 70 ine 1991-1995 yılları arasında, anterior girişim ve posterior enstrumantasyon uygulanmıştır. Anterior debridman sonrasında olguların yaklaşık 2/3 üne (81 olgu) otojen iliak kemik grefti, 113 üne (iki seviyeden fazla tutulum, 40 olgu) otojen fibula grefti ve kot greftleri konulmuştur. 30 olguda tüm kolon vertebra MR ile incelenmiş ve %10 olguda, direk grafilerde görülmeyen ve bazı olgularda nörolojik kayıptan sorumlu olan, birden fazla seviyede lezyonla karşılaşılmıştır.\nBulgular: Sadece anterior girişim yapılan 57 olgunun 21 inde ameliyat sonrasında 10° ve üzerinde kifoz artışıyla karşılaşılmıştır. Kifoz artışının 3 olguda greft kaymasına, 2 olguda greft rezorpsiyonuna, 16 olguda ise greftin spongioz kemik yatağına gömülmesine bağlı olduğu görülmüştür. Kombine girişim ve posterior enstrumantasyon uygulanan olguların hiçbirinde greft yetersizliği veya kifoz artışıyla karşılaşılmamıştır. Kifoz artışı açısından iki grup arasındaki fark istatistiksel olarak anlamlı bulunmuştur (p=0.047). Başvurduklarında paraplejik veya paraparezik olan 14 olguda, cerrahi tedavi sonrasında 1-6 haftalar arasında nöro-lojik iyileşmenin başladığı görülmüştür.\nSonuç: Omurga tüberkülozunun cerrahi tedavisinde altın standart anterior radikal debridman ve strut greft uygulamasıdır. Ancak immobilizasyon dönemini ve hastane kalış süresini kısaltmak, kifoz deformitesini iyi ve kalıcı şekilde düzeltmek, greft yetersizliğini ve omurgada çökmeyi engellemek için posterior enstrumantasyon ve füzyon gereklidir ve anterior radikal girişime daima eşlik etmelidir.
