In this paper the methods of internal fixation, other than AO osteosynthesis and Küntscher s nailing for the tibial shaft fractures are studied. In series of 60 cases the causes of skin neecrosis seen after internal fixation by plate and screws was investigated. Incidence of this skin necrosis was %33,33 after AO compression plate-fixation (4 out 12 cases) and % 6, 25 after application of ordinary plate fixation (3 out of 48 cases). This findings showed that incidence of skin necrosis is 5 times greater, if the AO plate is applied to the antero-medial surface of the tibia. Therefore application of this plate to the antero;lateral surface of the tibia is recommended.
Özet
Bu çalışmada, AO ve KÜNTSCHER osteosentezi dışında tibia diafizi osteosentezleri incelenmiştir. Ayrıca, tibia diafizine plak vida osteosentezi uygulanan 60 vak alık bir seride, AO osteosentezi ile diğer plak-vida osteosentezilerinden sonra, ciltte defekt yapan sekonder nekrozlar araştırılmıştır. 12 AO piak-vida ostsosentezinde 4 (%33,33) cilt nekrozu, 48 diğer plak-vida osteosentezinde ise sadece 3 (% 6,25) cilt nekrozu görülmüştür. Araştırmanın vardığı sonuç, AO plak-vida osteosentezlerinde, tibia nın ön iç yüzüne yapılan metal tatbiklerinde, diğer plak-vida osteosentezlerine göre 5 misli fazla bir oranda cilt nekrozu oluşmasıdır. Bu sakıncayı önlemek için, plak-vida uygulamalarının tibianın ön dış yüzünden yapılması önerilmiştir.
