The effects of low molecular weight heparin enoxaparin on the microsurgical vessel repair

Abstract

In this study, enoxiparin as a low molecular weight heparin was used to prevent thrombosis on the microsurgical vessel repair and the results were discussed in the light of the literature. The study was performed on femoral arteries of twelve rabbits. The cutting edge on the femoral arteries which was made paralel the long axis of the vessel about 1 cm was repaired with microsurgical techniques. Enoxaparin was administered 10 mg/kg peroperatively and continued the same dose once a day for 10 days. The function of repaired vessel was evaluated with muscle perfusion scintiphotography at the 10 th day and clinical observations. At the 10 th day rabbits were sacrified and the vessel was examined by histologic evolution for thrombosis and intimal hyperplasia. No functional problems for all repaired vessel were seen. No bleeding occured. Thrombosis was observed on two vessel and intimal hyperplasia on four. We concluded that enoxiparin is a useful antithrombotic agent, but not anticoagulant, for prevention to thrombosis on microsurgical vessel repair and for that reason enoxiparin can be used as a prophylactic treatment of patients undergoing reconstructive vascular surgery.

Bu deneysel çalışmada, düşük molekül ağırlıklı heparin preperatlarından olan enoxiparin, mikrocerrahi teknikle yapılan damar tamiri sonrasında trombüs oluşumunu engellemek için kullanıldı ve sonuçlar literatür ışığı altında tartışıldı. Çalışma oniki adet tavşanın femoral arterleri üzerinde yapıldı. Çalışmada, tavşan femoral arterlerinin ön yüzünde oluşturulan aner longitüdinal aksına paralel yaklaşık 1 cm lik kesiler mikrocerrahi teknikle tamir edildiler. Tavşanlara, ameliyat anından itibaren enoxiparin 10 gün süreyle, günde tek doz, İM, 10 mg/kg dozunda uygulandı. Damarların fonksiyonel işlevleri kas perfüzyon sintigrafisi ve klinik gözlem ile takip edildi. Cerrahi sonrasi 10. günde sakrifiye edilen tav sarıların femoral arterleri trombüs oluşumu ve intimal hiperplazi için histolojik açıdan incelendi. Tamir edilen damarların hiçbirinde fonksiyonel bir sorun saptanmadı. Hiç bir hayvanda kanama tablosu gözlenmedi. Trombüs iki olguda, intimal hiperplazi ise dört olguda tespit edildi. Düşük molekül ağırlıklı heparin preperarı olan enoxiparinin mikrocerrahi yöntemle yapılan damar tamirlerinde antikoagulan etki gösteremeden antitrombotik ajan olarak kullanılabileceği ve bundan dolayı da arteryel rekonstriksiyonlarda tromboz profilaksisi için tercih edilebileceği düşünüldü.
