Scaphotrapeziotrapezoid arthrodesis in the treatment of advancedstage Kienbock’s disease

Abstract

Objectives: We evaluated the results of treatment of Kienböck’s disease by scaphotrapeziotrapezoid (STT) arthrodesis and lunate excisional arthroplasty with a coiled soft tissue replacement.\nMethods: Nineteen patients (18 males, 1 female; mean age 23 years; range 20 to 55 years) with stage II or III Kienböck’s disease underwent treatment with SST arthrodesis and lunate excisional arthroplasty with a coiled soft tissue replacement. Fifteen (79%) and four patients (21%) had right and left-sided wrist involvement, respectively. The severity of the disease and clinical results were assessed according to the criteria by Lichtman et al. Radiologically, carpal height and postoperative osteoarthritic changes were examined. The mean follow-up was 30.8 months (range 12 to 87 months).\nResults: Moderate to severe pain observed preoperatively in all patients disappeared in eight patients. Moderate pain observed in five patients in the early postoperative period completely resolved at the end of the follow-up. Six patients continued to have both early and late postoperative pain. Grip strength increased markedly in 14 patients, remained unchanged in three patients, and decreased in two patients. The results were satisfactory in 13 patients (68.4%), while six patients (31.6%) were regarded as failure. Osteoarthritic changes at the radioscaphoid joint were observed in four patients, two of whom underwent subsequent wrist arthrodesis.\nConclusion: Scaphotrapeziotrapezoid arthrodesis maintains carpal height, with markedly reduced pain, improved grip strength, and preserved functional wrist mobility.

Amaç: İleri evre Kienböck hastalığı olan olgularda skafotrapeziotrapezoidal (STT) artrodez ve yumuşak doku replasmanı ile lunatumun eksizyonunun sonuçlarını değerlendirmek.\nÇalışma planı: Evre II ile evre III Kienböck hastalığı olan 19 olgu (18 erkek, 1 kadın; ort. yaş 23; dağılım 20-55) os lunatum eksizyonu+yumuşak doku replasmanı+STT artrodez yöntemi ile tedavi edildi. Hastalıktan etkilenen el bileklerinin 15’i sağ (%79), dördü sol el bileği (%21) idi. Hastalığın durumu ve klinik sonuçlar Lichtman ve ark.nın kriterlerine göre değerlendirildi. Radyolojik olarak karpal yükseklik oranı ve ameliyat sonrasında el bileğindeki osteoartritik değişiklikler incelendi. Ortalama takip süresi 30.8 ay (dağılım 12-87 ay) idi.\nBulgular: Ameliyat öncesi tüm hastalarda gözlenen orta ya da şiddetli derecede ağrı, ameliyat sonrasında sekiz hastada kayboldu. Beş hastada ameliyat sonrası erken dönemde görülen orta şiddette ağrı, geç dönemde geçti. Altı hastada ise hem erken hem de geç dönemde ağrı vardı. On dört hastada ameliyat sonrası dönemde kavrama gücünde belirgin artış oldu, üç hastada aynı kaldı, iki hastada ise azaldı. Sonuçlar 13 hastada (%68.4) tatminkar, altı hastada (%31.6) başarısız şeklinde değerlendirildi. Ameliyat sonrasında dört hastada osteoartritik değişiklikler gözlendi. Bunlardan ileri derecede osteoartrit gelişen iki hastada el bileği artrodezi uygulandı.\nÇıkarımlar: Skafotrapeziotrapezoidal artrodez ile karpal yükseklik korunurken, ağrı belirgin derecede azalır, kavrama gücü artar ve fonksiyonel el-bilek hareketleri korunur.
