Results of Grice operation in cerebral palsy patients

Abstract

In the Department of Orthopaedic Surgery and Traumatology, Ege University, Medical Faculty, 45 Grice-Green operations and adjuvant operations were performed to correct planovalgus deformity in 45 cerebral palsy patients. They were 14 males and 13 females, with minimum age 4, maximum age 11 , average age: 7, mean age 8. Follow-up period changed between 3 months and six years, with average one year. Grice procedure were perfomed on 12 feet alone, on 8 feet with heel cord lengthening, on 8 feet with peroneal lengthening, on 9 feet with heel cord lengthening and peroneal lengthening, on 6 feet with peroneal re-routing and 2 feet with heel cord lengthening and peroneal re-routing. Original technique with tibial graft and-wire insertion were used for stabilization. The results were excellent in 43 feet (%55.6), good in 18 feet (%40), poor in 2 feet (%4.4) High rate of satisfactory results were attributed to the preoperative good assessment and the combination of Grice procedure with the soft tissue operations.

Özet

Ege Üniversitesi Tıp Faktültesi Ortopedi ve Travmatoloji Ana Bilim Dalında 1982-1988 yılları arasında serepral felçli 27 hastanın 45 ayağında planovalgus deformitesini düzeltmek için Grice-Green ameliyatı ve ek ameliyatlar uygulanmıştır. Hastaların 14 ü erkek, 13 ü kız çocuğu, minimum yaş 4. maksimum 11, ortalama 7 , mean yaş 8 idi. Takip süresi 3 ay ve 6 yıl arasında değişti. ortalama 1 yıldı. Grice ameliyatı yalnız başına 12, aşiloplasti ile 8, peroneal uzatma ile 8, aşiloplasti ve peroneal uzatma ile 9, peroneal rerouting ile 6, aşiloplasti ve peroneal re-routing ile 2 defa beraber uygulandı. Orijinal tekniğe uygun olarak tibial grefle Kirschner teli ile tesbit yapıldı. 43 ayağın 25 (%55.6) inde çok iyi, 18 (%40) ında iyi, 2 ayakta (%4.4) fena sonuç alındı. Başarılı sonuç yüksekliği, ameliyat öncesi iyi değrlendirme ile, Grice ameliyatının yumuşak doku ameliyatları ile kombinasyonuna bağlandı.
