Abstract

Objectives: This study was designed to evaluate the clinical and radiographic features of neonatal septic arthritis, to investigate the factors facilitating its occurrence, and to determine the causative microorganisms.\nMethods: Fourteen patients (9 boys, 5 girls) with a diagnosis of neonatal septic arthritis were treated and monitored during a six-year period. All the patients were below two months of age. White blood count, Gram-staining and culture results of the synovial fluid from the affected joint were obtained and C-reactive protein and erythrocyte sedimentation rate were measured. Histories included premature birth (n=4), umbilical catheterization or venous cut down (n=3), sepsis (n=3), perinatal asphyxia (n=2), and difficult labour (n=1).\nResults: Blood and joint fluid aspirate cultures were positive in seven and eight patients, respectively. Staphylococcus aureus was the most commonly encountered organism. One patient died from sepsis. Deformation and septic dislocation of the femoral head were detected in one patient.\nConclusion: Early diagnosis and treatment of neonatal septic arthritis are essential to protect children from impending life-long disabilities

Özet

Amaç: Yenidoğan döneminde görülen septik artritli olguları klinik ve radyolojik olarak değerlendirmek ve artrit oluşumunu kolaylaştıran faktörleri ve etken mikroorganizmaları incelemek.\nÇalışma planı: Kliniğimizde altı yıllık sürede yenidoğan septik artriti tanısı ile 14 hasta (9 erkek, 5 kız) takip ve tedavi edildi. Olguların tamamının yaşları iki ayın altında idi. Eklem aspirasyon sıvısında beyaz küre. Gram boyama ve kültür, kanda ise sedimantasyon, C-reaktif protein değerleri incelendi. Dört olguda prematür doğum, üçünde umblikal kateterizasyon veya venöz cut-down, üçünde sepsis, ikisinde perinatal asfiksi, birinde zor doğum öyküsü vardı.\nSonuçlar: Yedi olguda kan kültüründe, sekiz olguda eklem aspirasyon sıvısında üreme saptandı. En sık üreyen mikroorganizma Staphyiococcus aureus idi. Olgulardan biri sepsisten dolayı kaybedilirken, bir olguda femur başında deformasyon ve septik dislokasyon saptandı. \nÇıkarımlar: Yenidoğan septik artritinde erken tanı ve tedavi çocukları yaşam boyu etkileyecek sakatlıktan kurtarabilecektir.
