Abstract
Twelve cases of delayed and non-union in factures of the neck of the femur in children were evaluated. Mean age of these cases was 11.4 with 8 girlsand four boys. The problem of union developed after treatment by bone setters in six cases, after conservative treatment in four, and after surgical treatment in two cases. \nSeven cases accepted surgical treatment. The surgical procedures were subtrochanteric osteotomy with bone grafting in three, internal fixation with Knowles pins and bone grafting in two cases. \nTwo cases whose fixation were made by Knowles pins failed to unite. One of these cases lost follow up. The unİon has been obtained İn the other case later by subtrochanteric osteotomy. We have concluded that, subtrochanteric osteotomy with bone grafting is treatment of choice in these cases.
Özet
Bu çalışmada, kliniğimizde 1978-1988 yılları arasında, çocukluk çağı femur boyun kırığı sonucu kaynama gecikmesi ve kaynamama saptanan 12 olgunun değerlendirilmesi yapıldı. Olguların 3 ü kız 4 ü erkek olup ortalama yaşları 11.4 idi. Kırık, 11 olguda travmatik, 1 olguda patolojik idi. Kaynama sorunu, 6 olguda sınıkçı tedavisi 4 olguda konservatif tedavi, 2 olguda cerrahi tedavi sonrası gelişmişti. Olguların 7 si cerrahi tedavi kabul etti. 3 olguda subtrokanterik osteotomi ile birlikte greftleme, 2 olguda Pugh çivisi ile çivileme ile birlikte grefteleme, 2 olguda Knowles çivileri ile çivileme ile birlikte greftleme ameliyatları yapıldı. Knowles çivileri ile çivileme yapılan 2 olguda kaynama olmadı. Kaynama olmayan olgulardan biri takipten çıktı. Diğer olguda daha sonra subtrokanterik osteotomi yapılarak kaynama elde edildi. Böylece 6 olguda kaynama sağlandı. Bu tür olgularda, subtrokanterik osteotomi ile birlikte greftleme yönteminin daha uygun olduğu sonucuna varıldı.
