Comparison of the effect of perineural and systemic morphine administrations on postoperative analgesia after axillary block

Abstract

The pain free postoperative period of a patient, operated on the upper extremity using axillary blockage, is directly related to the effective duration and quality of the local anesthetic agent used. To compare the effects of perineural morphine administration which is performed mixture of morphine into the local anesthetic solution, with the same amount of systemic morphine use (intramuscular), we studied 50 patients who are operated using axillary blockage ASA classification of I-II and with between the ages of 15-65 at the Department of Orthopaedics and Traumatology, University of Istanbul. As a result, we concluded that perineural blockage improves post-operative analgesic quality compared to the same amount of morphine, injected by the intramuscular route.

Özet

Brakiyal pleksus un aksiller blokajı ile yapılan üst ekstremite operasyonları sonrası hastanın post-op dönemde ağrısız bir periyod geçirebilmesi verilen lokal anestetiğinin etki süresi ve kalitesi ile direkt olarak ilgili-dir. Çalışmamızda İstanbul Tıp Fakültesi Ortopedi ve Travmatoloji Anabiiim Dalı ameliyathanelerinde ASA sınıflaması I-II, yaş ortalaması 15-65 olan 50 hastaya aciller blok sırasında lokal anestetik solüsyona morfin eklenmesi ile yapılan perinöral morfin uygulamasınm aynı dozda morfinin sisfemik (im) kullanımınm post-op ağrı üzerine etkileri karşılaştırılmıştır. Sonuç olarak 6 mg morfinin aksiller blok sırasında lokat anestetiğe ilave edilerek perinöral uygulamasının aynı dozda morfinin im. yolla kullanılmasına oranla post-op analji katitesini arttırdığı kanısına varılmıştır.
